First, let me emphasize that I am not a gynecologist or reproductive endocrinologist, so not exactly an expert on polycystic ovary, also known by an older generation as Stein-Leventhal syndrome.

It is true that these women have high testosterone levels, but their physique is anything but athletic. They are generally overwight, tend to be diabetic, so not considering them in the same class as the transsexuals is entirely appropriate.
